Risk Assessment Record

To create a Risk Analysis, an Assessment Type with predefined level definitions must be registered. Fields marked with an asterisk (*) are mandatory for record creation and proper system functionality.

This section captures the essential details of the risk assessment, including:

  • Risk Assessment Name: Specifies the title of the risk assessment, providing a clear reference for identification and tracking.

  • Risk Assessment Number: A unique identifier assigned to each risk assessment. The combination of Number and Revision must be unique.

  • Revision: Represents the version of the risk assessment. Each revision documents modifications or updates while maintaining the original Risk Assessment Number for continuity.

  • Assessment Type: Specifies the criteria for the risk assessment based on the Risk Assessment Type Definition. It determines the rankings and evaluation methodology. Only active records are available. If the desired value is not listed, click the external link icon to access the related form and add a new entry.

  • Status: Indicates the current stage of the risk assessment (e.g., Open, Under Review, Approved, Closed, Canceled, or Duplicated).

  • Description: Summarizes the risk assessment, covering the nature of the risk, triggering events, impacted areas, consequences, root causes, and exposure, ensuring clarity on the assessment scope.

Creation & Storage

By default, the assessment is assigned to the user who initiated it, referred to as the Owner. Ownership can be reassigned if needed. If the value you want to add doesn’t appear in the list, click the external link icon to navigate to the related form. This section also includes storage-related details and record management functionalities:

  • Created By: Automatically assigned to the user who initiates the record.

  • Created Date: Records the exact timestamp when the record is created.

Some fields, such as Created By and Created Date, are system-generated to ensure traceability, recording both the initiating user and the exact timestamp of creation.
